This assumes you've gone and bribed Igor with scumble, and found the saucepan (search the dresser upstairs if you don't have one yet). You'll also want an empty shot glass for measuring. And remember: you can take extra dry ingredients out again, but you can't get liquids out the same way.
To make the fatsup, I used beer and brandy from the booze stall in OC, water from a sink, and 5 spicy sausages from the Mad Wolf shop and the restaurant on Long Street in BP. If you haven't found the flour and lard yet, look around the other buildings within a few blocks of Thomas'.
I'm putting these in by command because it can get confusing otherwise.
"fill pan 1/3 full from water"
"fill pan 2/3 full from beer" (this should give you watery beer)
"put 4 pinches garlic in pan" (garlicky watery beer)
"put 1 handful flour in pan"
"put 18 pinches flour in pan" (beer and garlic gravy)
"put lard in pan"
"get lard from pan" (greasy beer and garlic gravy)
"fill shot glass 1/100 full from brandy" (this should give you 1 drop of brandy in the glass)
"pour glass into pan" (repeat until you get something like alcoholic gravy?)
Now all that's left is the sausage. Put the sausages in the slicer, "slice all in slicer", and add 4 sausages worth of slices plus one slice to the pan. This should give you about a pint of "uncooked fatsup".
Put the pan on the stove, and turn the stove up and down to keep it bubbling at the right temp ("check temp on stove"). It takes a few minutes to cook, and will need to have the temp adjusted a few times during the process. It does give you warnings that the temp needs changing. When it's finished, give the pan to Thomas.
