There are a few different ways.
Gapp priests have a ritual to mend clothes.
Placing the item on the low altar in Gapp's temple in AM will use your faith gp to mend clothes. You may have to place the item of the altar several times. You can tm fa.po but you spend a lot of time just standing around waiting for your gp to regen so you can place the item on the altar again.
There is a tailor in Bes Pelargic that will mend clothes for a small fee.
There is a seamstress in Djelibeybi that will mend clothes for a small fee.
There used to be a NPC in the magic emporium on Sator Square in AM that would mend wizards' clothing for a small fee. Don't know if it is still there, haven't tried that one in years.
You can use a small silver needle and thread, available in Nicolette's shop in the basement of the thieves guild and other places around the Disc. Hold the needle, thread the needle with the thread and sew the item of clothing. It uses some cr.ma.ne skills. I prefer this method because you can do it on the fly and the clothing never leaves your possession in case the mud crashes.
If your clothing is leather, you will probably need to use the leatherwork command in a tannery. It uses some cr.ma.ta and cr.ma.le skills.
